The inspector’s account

On 06/04/26, Licensing Program Analyst (LPA) Regina Cloyd conducted an unannounced annual visit using the CARE Inspection Tool. LPA met with Administrator Jennifer Block and explained the purpose of today’s visit. The facility is licensed to serve two non-ambulatory and four ambulatory residents. The facility has a hospice waiver for three residents. Two residents are currently receiving hospice services.

The facility is a two story structure located in a residential neighborhood. The first-floor licensed facility consists of the following: four resident rooms, three bathrooms of which two are private, living room, dining room, kitchen, office and an enclosed patio/activity area.

The Administrator accompanied LPA inside and outside the facility during this inspection. Outside grounds were toured and no bodies of water were observed. Walkways around the facility were clear of hazards.

Resident bedrooms had bed linens and closet/drawer space to accommodate each resident comfortably. There are no security bars or weapons on the premises. Resident bathrooms were checked.
