The inspector’s account

On 05/18/2023, Licensing Program Analyst (LPA) Alfonso Iniguez conducted an unannounced annual required visit with a primary focus on using the new CARE Inspection Tool. LPA met with administrator Gary Strathearn and staff Svetlana Dagala and explained the purpose of today’s visit. The facility is licensed for (6 residents), (2) non-ambulatory, (4) ambulatories and Hospice Waiver for (3). Currently, there are 3 residents, all are over the age of 60.

The facility is a one-story structure located in a residential neighborhood. The first-floor licensed facility consists of the following: four (4) resident rooms, three (3) bathrooms, living room, dining room, kitchen and enclosed patio/activity room area.

LPA and Administrator toured the physical plant. There were no bodies of water or obstructions on the premises. Beds and bedding supplies were in good condition, adequate lighting provided, storage for client personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. Bathrooms were found to be within Title 22 regulations and were clean and operational. The water temperature ranged from 110.5F° – 114.2F°. The room temperature ranged from 76F° – 78F°.
