On 07/27/26, Licensing Program Analyst (LPA) Regina Cloyd conducted an unannounced case management - incident visit and met with Administrator Stephanie Brown. The purpose of the visit is to follow up on Client #1 (C1) Death Report that the Department received on 07/26/26 7:12 AM.
During today's visit, The Department obtained Staff and Client Roster, Visitor Sign In Log, Client #1 Record, and video recordings (email links).
An exit interview was conducted and a copy of this report was provided to the Administrator Stephanie Brown.
