Facility Evaluation Report
On 09/22/2025, Licensing Program Analyst (LPA) Elvira Gonzalez conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Selina Du, Client Care Technician, and the purpose of the visit was explained. Kari Wood, Associate VP of Operations, met with LPA shortly after. The facility is a short term crisis residential, approved for (6) ambulatory individuals.
The facility is a three-story home located in a residential neighborhood. The property consists of the following: the first floor consists of a nurse/medication room, therapy room, kitchen, activity/lounge area, arts activities area, living room, dining room, (1) common bathroom, (2) staff bathrooms. The second floor consists of a staff office/supply room, (3) client bedrooms of which each has their own private bathroom, a laundry room, and (1) common bathroom. The third floor is a sitting/lounge area with a balcony. There is an outside shaded area with seating in the backyard.
LPA Gonzalez and Kari Wood toured the inside and outside of the facility. All client rooms were checked. Mattresses and box springs were in good condition, and plenty of dresser and closet space was observed. Walls and floors were clean and in good repair. Bed linens, comforters and bath towels were adequately stocked at the time of visit. Bathrooms were found to be within Title 22 regulation. Toilets and water faucets worked properly. Shower was free of mold/mildew, there is adequate lighting, and sufficient toiletries accessible to clients. The water temperature properly measured between 105.0 F and 120.0 F.
Perishable and non-perishable food supply was checked and adequately stocked at time of visit. Smoke and carbon monoxide detectors were observed and operational. Fire extinguishers were fully charged. Toxins, knives and other sharp objects were locked and inaccessible to clients. Medications were centrally stored and properly locked, first aid kit along with manual was checked and fully stocked. Outside grounds were toured and no bodies of water were observed. Exits/ Walkways around the home were free of debris and hazards.
