Licensing Program Analyst (LPA) Alberto Lopez made unannounced visit to conduct health and safety check. LPA met with Maria Ortiz,LCSW, and LPA discussed purpose of the visit.
LPA reviewed and obtained C1 face sheet, physicians report, two (2) incident reports dated 08/10/2025 regarding C1, C1 discharge summary and information of current location for C1.
LPA interviewed Maria Ortiz, LCSW and took a tour of facility with Maria Ortiz, LPA did not observe any health or safety hazards during this visit.
No deficiencies cited. Exit interview held with Maria Ortiz, LCSW and copy of report provided.
