Facility Evaluation Report

Licensing Program Analyst (LPA) Sakinah Madyun conducted an unannounced continued Annual Inspection visit on 09/11/25 at approximately 10:15am. LPA was met by Assistant Program Director Rosa Arroyo and explained the reason for the return visit. The facility is an Adult Day Program (ADP) licensed to serve a capacity of 55 Ambulatory Developmentally Disabled adults in the age range 18 and over, of which 25 may be Non-Ambulatory.

During today's continued annual visit, LPA Madyun conducted five (5) staff and five (5) client interviews.

Inspection tool completed. No deficiencies are being cited today. Exit interview conducted with Rosa Arroyo and a copy of this report was provided.
