The inspector’s account

Licensing Program Analyst (LPA) Sakinah Madyun conducted an unannounced annual visit on 08/12/25 at approximately 9:00am using the Care Inspection Tool. LPA was met by Program Director Diane Perez and Assistant Program Director Rosa Arroyo and explained the reason for the visit. The facility is an Adult Day Program (ADP) licensed to serve a capacity of 55 Ambulatory Developmentally Disabled adults in the age range 18 and over, of which 25 may be Non-Ambulatory.

Both the Program Director and Assistant Director accompanied LPA on the tour of the single-story facility and LPA observed the following:

There are four (4) staff offices, lobby, a conference room area, five (5) activity rooms, four (4) client restrooms, of which one (1) has a showering station, and one (1) has a changing bed station. One (1) staff restroom, a copy machine room, a nurse's room, relaxation room, and kitchen area with a dining room area. During the tour LPA observed: a total of four (4) clients and four (4) staff were at the facility during the visit. The staff to client ratio is 1:1.

The Day Program site is clean, safe, sanitary and in good repair. All passageways are free from obstruction. Disinfectants such as cleaning solutions and poisons are inaccessible to clients, and are locked and secure in the storage closet or cabinets. The facility has five (5) full green marked fire extinguishers, carbon monoxide detectors, and a sprinkler system in place throughout, and last inspected 12/26/24. All four (4) restrooms were observed to be clean and have the required grab bars for non-ambulatory clients. All water temperatures tested between 110.0 to 118.6 degrees Fahrenheit, which is within the required 105-120 degrees Fahrenheit. Continued LIC 809C
