Facility Evaluation Report

On 2/24/2026, Licensing Program Analyst (LPA) Bernadette Allen conducted an unannounced visit to conduct a health and safety check and obtained documents. LPA identified herself and met with Maria Bravo-Galvan, who was informed of the purpose of the visit.

At 2:45 PM, LPA Bernadette and Cody Wagner conducted a tour of the facility, which included the lobby, 1st and 2nd floors. During the tour, LPA did not observe any health or safety concerns.

LPA requested and was provided the following information/documentation:

Fire Watch Log from Wednesday 2/17/2026–2/24/2026, and a copy of the Los Angeles Fire Department Fire Protection Equipment Performance Report – Re-Test date 2/5/2026.
