Facility Evaluation Report
On 02/18/2026, Licensing Program Analyst (LPA) Villegas conducted a case management Health Check visit. LPA met with Administrator, Maria Galvan as the purpose of the visit was explained.
During the visit LPA collected and reviewed fire watch logs dated 02/05/2026 through 02/18/2026, record review revealed that fire watches have been conducted every hour by two staff members. Fire Watch logs are dated and document the start time, end time, conditions observed, issues noted if any, and are initialed by both staff members.
No deficiencies were observed during the visit.
An exit interview was conducted, and a copy of this report was provided to the Administrator Maria Galvan.
