The inspector’s account

On 03/13/26 Licensing Program Analyst (LPA) Villegas conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Cheryl Cambay as the purpose of today’s visit was explained. The facility is licensed to serve adults ages 60 and over. Approved for (6) non-ambulatory residents, of which (1) may be bedridden in bedroom #1. There is an approved hospice waiver approved for (2) residents. Facilities fees are current, liability insurance is active (James River Insurance Company 00141167-3 exp: 03/06/27).

The facility is a single story home that consists of the following: A kitchen, dining room, a breakfast nook, living room, (4) Bedrooms, (2 1/2) Bathrooms, backyard, and a patio with shaded area, LPA observed ramps on the back porch and north side of the home, indoor and outdoor activity area, laundry room, and an attached garage. Bedrooms were checked, mattresses and box springs were in good condition, adequate lighting, plenty of dresser and closet space was observed. Bathroom toilets and water faucets worked properly, shower was free of mold/mildew, and there are sufficient toiletries accessible to residents. The water temperature properly measured between 114.7 F.. A supply of perishable and non-perishable food was observed, toxins and knifes were stored and inaccessible to residents, no weapons nor bodies of water on the premises, exits and walkways are free of debris/hazards.

LPA conducted a records review of (3) staff records, (5) resident records, and (5) medication administration records, no discrepancies observed. Facility does not handle resident's cash resources. Medications were centrally stored and properly locked, (3) first aid kit was checked and fully stocked. The last fire was conducted on 12/27/25, 1 fire extinguisher fully charged, carbon monoxide and smoke detectors are operational. There is a land line at the facility.
