Facility Evaluation Report

On 07/28/26 Licensing Program Analyst (LPA) Mario Leon conducted a case management visit to deliver one (1) deficiency, related to California Department of Social Services (CDSS) observation on 04/23/26. CDSS has observed that one (1) resident (R1) did not have an annually updated Physician's report.

There has been one (1) deficiency delivered during today's visit.

An exit interview was held with Holly Rice - Executive Director and a copy of one (1) deficiency, facilities' appeal rights and this report have been provided.
