Facility Evaluation Report
On October 31, 2024, Licensing Program Analyst (LPA) Deborah Lee visited this facility for an announced Pre-Licensing visit. LPA met with Josefina Guiao Applicant/Administrator and together we toured the house/facility inside and out. The home is a single-story dwelling with manicured front yard on property. The home consists of: 4 bedrooms, 2 bathrooms, living room, formal dining area, kitchen, laundry room (located off of the kitchen). There is a detached garage used for storage. There is an outside/patio covered area with ample space for outdoor activities.
The following was observed, reviewed and inspected: Upon arrival, LPA observed required postings: clients right, facility sketch, see something say something poster, emergency disaster plan. Bedrooms 1-4 are all clean safe and sanitary. Bedrooms 1-4 are private rooms and will be occupied by 1 client. All bedrooms contain the required about of furniture and lighting fixtures and personal storage space, all beds have the required amount of linen, mattresses and box springs are in good repair, bathrooms were clean and operational, water temperature tested within normal limits (between 105-120 degrees F ). LPA observed locked areas for resident files, medications, toxins and sharp objects are inaccessible to residents in care. Smoke detectors were combo smoke and carbon monoxide model which were tested and functioning—there are a total of (8) smoke detectors/carbon monoxide comb in the home. LPA observed ample supply of clean linen located in a hall closet. There are 2 fully charged fire extinguisher located in the dinning and entry entryway of the facility. LPA observed a 7-day supply of nonperishable foods and 3-day supply of perishable foods, 1 fully stocked first aid kit with tweezers, scissors, band aid, gauze, antibiotic ointment and manual, all exits are marked, hallways and passageway are clean and clear of clutter and obstructions. The front yard and back patio area are well maintained. LA City Fire Department granted on 04/30/24 with approval for (4) 18 through 59 ambulatory residents 3 may be non-ambulatory.
