Complaint Investigation Report
Based on the LPA's investigation, the investigation revealed the following. For Allegation 1 – Resident was provided illegal drugs while in care. Interviews with Ken and Josie, both agreed that C1, was at the facility over a year ago. C1 left the facility around December 2021. C1 had broken the room in C1’s room, the facility called the police and the police put a 5150 hold on C1. C1 was taken to the hospital and C1’s family member was informed. The facility did send the SIR dated 12/23/21, to LPA at the time of the incident, where C1 broke the window. Ken, House manager stated that C1 has not been back to the facility for over a year, they were never any reports of any illegal drugs given or found on C1. About 2/3 weeks ago, family member called facility to see if C1 could return to the facility. House manager Ken advised Family member, that it would be better if C1 was put somewhere that had a higher level of care, Family member got upset and threatening Ken. Interviews with C#2 – C#8, they all stated that they have never been offered drugs by staff or any other client. LPA reviewed C1 records, Facility evaluation- dated 06/17/21, and Physician’s report – dated 05/15/20, did not reveal any drugs abuse. There were no previous reports for C1, were they complained or reported C1 being given or offered any drugs by staff or clients. The interviews and records reviewed did not concur with the above allegation.
Allegation 2 - Resident was not properly supervised while in care. Interviews with Ken and Josie, both agreed that C1, was at the facility over a year ago. C1 left the facility around December 2021. They also stated that they never had any complaints from C1 family before about not taking care of C1 properly. Family member was just mad because the facility did not want to accept C1 back. Interviews with C#2 - C#8, stated that they feel they are being supervised and they like the facility. LPA reviewed C1 file, there were no SIR’s that related to any complaints or incident that had to do to with C1 not being properly supervised. The interviews and records reviewed did not concur with the above allegation.
