Facility Evaluation Report
Licensing Program Analyst (LPA) Galarza conducted a Case Management- Incident visit to follow up on a SOC 341 Report of Suspected Dependent Elder Abuse incident report dated 5/15/2026. LPA explained the purpose of the visit to LVN Andrea Lopez. RN Sally Wu arrived shortly after.
According to SOC 341, on May 17, 2026, resident (R1) disclosed to a family member that a male staff was going to choke the resident. During today's visit staff (S1-S5) and resident (R1) were interviewed. The alleged staff is currently suspended pending facility internal investigation.
Based on interviews, resident (R1) is able to communicate but sometimes says inappropriate things to staff and has history of saying he is choking. Staff said that the resident has had choking incidents while eating. Per record review, R1 has self injurious behaviors, aggressiveness, and history of story fabrication. LPA did not observe any marks on R1's neck area. Resident (R1) was unable to answer questions regarding the alleged choking incident.
The facility conducts weekly RN assessments and daily body check assessments. There has not been any observations of physical abuse. A psychiatrist will be consulted for an evaluation. Behavior Report Plan will be reviewed and updated.
There is no indication of alleged physical abuse.
An exit interview was conducted with Sally Wu. A copy of the report was provided.
