Facility Evaluation Report
Licensing Program Analyst (LPA) Galarza conducted an unannounced Required- 1 year visit. The purpose of the visit was explained to Administrator John Nguyen. The facility is licensed as a level 6 Adult Residential Facility (ARF) serving developmentally disabled residents 18-59 years vendored by Eastern Los Angeles Regional Center.
Infection Control: The Infection Control Plan includes Environmental cleaning and disinfection activities.
Physical Plant/Environment Safety: The facility is a two-story home located in a residential neighborhood. It consists of four (4) resident bedrooms, (1) staff room, 3 bathrooms, kitchen, dining room, laundry room, living room, family room, patio area, backyard, and attached garage. Cleaning supplies, knives, and toxic substances are inaccessible to residents. Water temperature readings measured within the required 105 - 120 degrees Fahrenheit. The facility has a 1st Aid Kit and manual. Smoke, carbon monoxide detectors, and fire alarm were tested and are operational. The facility has two (2) fire extinguishers, fire sprinklers, and a fire pull alarm. Water temperature readings measured within the required 105 - 120 degrees Fahrenheit. Exit doors are free of any obstruction and there are no pools or large bodies of water.
Operational Requirements: Fire clearance is approved for six (6) non- ambulatory residents. Care and supervision to meet the clients needs was observed. No Special equipment or supplies are used by residents. The Surety Bond expires 5/29/2026.
Staffing: A total of 8 staff members provide care and supervision to the clients.
