Facility Evaluation Report
Licensing Program Analyst (LPA) Galarza conducted an unannounced Required- 1 year visit. The purpose of the visit was explained to Administrator John Nguyen. The facility is licensed as a Specialized level 4B Adult Residential Facility (ARF) that serves developmentally disabled residents ages 59 and under vendored by Eastern Los Angeles Regional Center.
Infection Control: The Infection Control Plan includes Environmental cleaning and disinfection activities. Facility has sufficient Personal Protective Equipment.
Physical Plant/Environment Safety: The facility is a two-story home located in a residential area licensed for 6 non- ambulatory residents. It consists of four (4) client bedrooms, (1) staff rooms, 3 bathrooms, kitchen, dining room, laundry room, living room, family room, patio area, backyard, and attached garage. Exit doors are free of any obstruction and there are no pools or large bodies of water. Smoke and carbon monoxide detectors were tested and are operational. The facility has two (2) fire extinguishers. Water temperature readings measured within the required 105 - 120 degrees Fahrenheit. The facility has a 1st Aid Kit and Manual. Cleaning supplies, knives, and toxic substances are inaccessible to residents.
Operational Requirements: Fire clearance is approved for six (6) non- ambulatory residents. Care and supervision to meet the clients needs was observed. No Special equipment or supplies are used by residents. The Surety Bond has an effective date of 2/28/2025. However, P & I ledgers show the amount safeguarded exceeds the facility's bond amount. A citation was issued.
Staffing: A total of 9 staff members provide care and supervision to the clients.
