Facility Evaluation Report
Licensing Program Analyst (LPA) Blanca Gonzalez conducted an unannounced required annual inspection visit. LPA was greeted by staff, and the purpose of the visit was explained. Administrator Indira Bhavsar arrived shortly after.
Facility is licensed to serve (6) developmentally disabled adults, ages 18-59. One (1) may be non-ambulatory. The facility is a one-story family home located in a residential area of San Dimas. The home consists of a living room, dining area, kitchen, three (3) client bedrooms, two (2) bathrooms, laundry room, front yard, backyard, and attached garage.
LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:
Cleaning supplies and toxic substances are inaccessible locked in cupboards in hallway. There are no firearms or weapons stored at the facility.
Kitchen was observed clean and free of pests. LPA observed sufficient supply of nonperishable food for one week and perishable foods for a minimum of two days. Soaps, detergents, and cleaning supplies were observed to be stored away from food supplies. Freezer were observed to be clean and within temperatures of 0-degree F and refrigerator with maximum temperature of 40-degree F.
