Facility Evaluation Report
Licensing Program Analysts (LPA) Elizabeth Irra and Sakinah Madyun conducted the required annual inspection. LPA met with Elizabeth Mendoza and discussed the purpose of today’s visit. Rodora Ilao arrived shortly after.
The program is licensed to serve (30) Developmentally Disabled adults. There is a fire clearance approved for (20) non-ambulatory clients, of which (6) may use wheelchairs and (4) may use walkers only. Hours of operation M-F 8A.M. to 3P.M.. All clients in this program receive case management services provided by San Gabriel Pomona Regional Center.
LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:
Infection Control: Facility has an Infection Control Plan in place.
Physical Plant & Environment Safety: Smoke alarms and carbon monoxide observed, tested and operable. Fire extinguishers observed (last serviced on 06/03/25). Last fire drill was conducted on 03/07/25. Hot water temperature measured the between 113.0* to 115.1*. Cubbies for clients to store their belongings were observed. Water fountain with filtered water observed.
Operational Requirements: Staff are adhering to operational requirements.
