Facility Evaluation Report
Licensing Program Analysts (LPA ) Blanca Gonzalez arrived at the facility to conduct an unannounced Annual Continuation visit. The initial visit was conducted on 03/19/26 . LPA met with Talitha Pagler LVN and the purpose of the visit was explained.
LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:
Client Records/Incident Reports: Four (4) client files were reviewed and were observed to contain Individual Health Care Plans (IHCP), IPP, medical assessments, admission agreements, personal rights, medical consent, and TB clearance.
Incident Medical and Dental: All residents have updated consultant assessments, Physician Reports, and physician orders.
Disaster Preparedness, and Emergency Intervention: Current Emergency Disaster Plan was not posted. Administrator had an electronic plan and agreed to email LPA updated plan. Technical Violation issued. First Aid Kit and Manual were observed. The last emergency drill was conducted on 02/02/26.
Per California Code of Regulations, Title 22, and California Health and Safety Code, the deficiencies observed during the visit are documented on the LIC809D. A civil penalty for $100 per day for a maximum of 5 days, for each staff member, for criminal record clearance violation is being assessed on the attached LIC421BG.
