The inspector’s account

Licensing Program Analyst (LPA) Blanca Gonzalez conducted an unannounced required annual visit. LPA was greeted by staff and the purpose of the visit was explained. Administrator Anahi Martinez arrived shortly after. The facility is an Adult Residential Facility for Persons with Special Health Care Needs (ARFPSHN) licensed to serve adults with developmental disabilities who are medically fragile and require 24-hour licensed nursing support. Clients receive services from San Gabriel/Pomona Regional Center.

LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:

Infection Control: The facility has a current Infection Control Plan.

Physical Plant/Environment Safety: The facility is a single-story home that contains five (5) bedrooms fully equipped with mechanical lifts, four (4) bathrooms of which one (1) is equipped with a mechanical lift; a living room, kitchen, dining area, laundry room, backyard with a shaded patio area, and attached garage.

The interior and exterior physical plant was inspected. Exit doors are free of any obstruction and there are no pools or large bodies of water. Smoke and carbon monoxide detectors are located throughout the facility. The facility has fire extinguishers throughout the facility, last serviced 01/23/2026. Water temperature was tested in bathrooms and measured 105°F which is within the required 105° F- 120° F.
