Facility Evaluation Report
Licensing Program Analyst (LPA) Cynthia Chan conducted the unannounced annual inspection on 1/9/2026. LPA met with the House Manager, Laura Soto, and explained the purpose of the visit. Administrator, Eileen Singh, arrived shortly after. The facility is licensed for 4 ambulatory clients only. There are currently 4 clients residing at the home, and they were placed by the San Gabriel/Pomona Regional Center.
The facility is a single story home consisting of 4 client bedrooms, 2 bathrooms (1 of which is located in bedroom #3), living room, dining area, kitchen, and an attached garage. The backyard has a swimming pool that is drained and covered and has a 5 feet fence on the perimeter. Each client bedroom has the required furniture such as a bed, a night stand, chair, and closet space. There are hallway lights. The facility has smoke and carbon monoxide combo detectors in each bedroom and hallways. The hot water temperature was measured within the required range of 105-120 degrees F. Knives and sharps are locked in a safety box. Cleaning supplies and detergents are locked in the garage cabinet. Extra linens and hygiene supplies are available. Food supplies of 2 day perishable and a week of non-perishable are observed. LPA reviewed 3 personnel files. The administrator's certificate expires on 4/9/26 and the TB & HIV training certificate was issued on 3/16/24. Staff have current CPR & First Aid certificates. LPA reviewed all 4 client files and their medications. The files have the require documents, however, the medical assessments did not have the TB results marked for 1 client. Emergency Disaster Plan and Infection Control Plan have been updated and reviewed annually. A technical advisory was given for the quarterly drills.
A deficiency was issued today. A copy of this report and appeal rights were given to the administrator.
