Facility Evaluation Report

Licensing Program Analyst (LPA) Cynthia Chan conducted the unannounced annual inspection on 3/4/25. LPA met with Staff, Arnulfo Ochoada. The purpose of the visit was explained. Administrator, Rachel Sabillo, arrived shortly after to assist. The facility is licensed to serve 4 ambulatory only adults ages 18 through 59. The home is vendorized by the San Gabriel/Pomona Regional Center. There are currently 2 clients residing at the facility.

LPA inspected the facility using the Compliance and Regulatory Enforcement (CARE) tool and observed the following:

The single story facility consists of 3 client bedrooms, 2 bathrooms (1 common and 1 jack and jill), living room, dining area, kitchen, and a garage used as an office. There are smoke and carbon monoxide combo detectors throughout the home. Facility has sufficient food supplies and foods are properly stored in the refrigerator. Knives and cleaning solutions are locked and inaccessible to clients. The hot water temperature is measured within the required range of 105-120 degrees F. Staff are continuing to clean and disinfect throughout the day.

There are 2 ambulatory clients residing at the home. LPA reviewed files for both clients and the required documents are maintained in their files. Medications were reviewed and are being given as prescribed by the physician. There are current no clients with any restricted or prohibited health conditions.

LPA reviewed the administrator and 2 other staff files. Administrator's certificate expires on 11/24/25. Staff files contain the health screening with TB test and current CPR & First Aid certificates. The Emergency and Disaster Plan is reviewed annually and drills were conducted. However, LPA advised administrator that the drills must be conducted at least quarterly for each shift.

No deficiencies were issued today. An exit interview was held and a copy of this report was given to the Staff Savong.
