Facility Evaluation Report

Licensing Program Analyst (LPA) Cynthia Chan conducted the unannounced annual inspection using the Compliance and Regulatory Enforcement (CARE) tools. LPA met with Staff and explained the purpose of the visit. The administrator Rebecca Reichenberger arrived shortly thereafter. The facility is licensed for six (6) ambulatory adults, ages 18 to 59. The home is vendorized by the San Gabriel/Pomona Regional Center.

The facility has 3 shared client bedrooms, 1 staff room, 2 bathrooms, living room, dining area, family room, kitchen, and attached garage. Client bedrooms are equipped with the appropriate furniture and storage space. There are extra linens and hygiene supplies. Facility has operable smoke detectors and a carbon monoxide detector. Knives and cleaning solutions are locked. The backyard has a shaded area that is furnished. The hot water temperature was measured between 105-120 degrees F. Sufficient food supplies of 2 day perishable and a week of non-perishable observed. There are no items obstructing the walkways.

LPA reviewed (3) staff files and files are complete. Administrator's (Rebecca Reichenberger) certificate expires on 9/9/27 and the HIV & TB certificate was issued on 6/17/25 . Staff files have current CPR & First Aid certificates and receiving appropriate annual training. Staff do not use any manual restraints on clients. LPA reviewed (5) client files and all the files contained the required documents. (3) out of the (5) residents taking medications are given as prescribed. The facility has the updated emergency and disaster plan and is conducting quarterly disaster drills.

No deficiencies issued today. An exit interview was held and a copy of this report was given to the administrator.
