Facility Evaluation Report

Licensing Program Analyst (LPA) Cynthia Chan conducted a case management visit due to a death of Client #1. LPA met with Staff and explained the reason for the visit.

On 6/18/26, Licensing received a death report for Client #1 (C1). The facility reported that on 6/16/26, C1 exhibited unsteady gait and difficulty ambulating. C1 was transported to the hospital for evaluation. C1 passed away at the hospital on 6/17/26.

During the visit today, LPA conducted a health and safety check and did not observe any concerns. LPA interviewed three (3) staff regarding the death of C1. Staff stated that they are aware of client's condition due to having Stage 5 kidney disease. The physician informed staff about C1's decline and was placed on palliative care.

LPA obtained copies of C1's documents and requested for licensee to send a copy of the death certificate once obtain.

An exit interview was held and a copy of this report was given to the administrator.
