Facility Evaluation Report
Licensing Program Analyst (LPA) Cynthia Chan conducted the unannounced annual inspection on 8/22/25. LPA met with Staff, Sylvia Ray, and explained the reason for the visit. The facility is approved for four (4) clients, ages 18 through 59, of which two (2) may be non-ambulatory. There are currently three (3) clients residing at the home and are receiving services through the San Gabriel/Pomona Regional Center.
LPA utilized the Compliance and Regulatory Enforcement (CARE) tools to inspect the facility.
The facility consists of 4 client bedrooms, 1 administrative office, 2 bathrooms, living room, open dining room and kitchen, and a laundry area. There is no swimming pool on the premises. There are no obstructions to the walkways. The fireplace is not in use. Sufficient food supplies of two days of perishables and a week of non-perishables are observed. Knives and cleaning supplies are locked. The hot water temperature was measured within the range of 105-120 degrees F. The backyard has a covered area with a table and chairs for clients to use. Extra hygiene supplies and linens were observed.
Medications are centrally stored and locked in the kitchen area. Staff are administering the medications as prescribed. LPA reviewed 3 staff files and 3 client files. Administrator's (Tomasa Coronado) certificate expires on 5/19/26. The HIV and TB certificate was issued on 7/19/24. Staff have valid CPR & First Aid certificates and receiving on-going training. Client files have the required documents. There are no clients with a restricted health condition. The Emergency Disaster Plan is being reviewed annually and facility is conducting emergency drills at least quarterly for each shift.
No deficiencies were observed today. An exit interview was held and a copy of this report was given to the administrator.
