Facility Evaluation Report
Licensing Program Analyst (LPA) Cynthia Chan conducted an annual inspection using the Compliance and Regulatory Enforcement (CARE) tools. LPA arrived unannounced and met with Ann Fernandez. The purpose of the visit was explained. The license is approved for four (4) ambulatory adults, ages 18 to 59. There are currently (4) clients residing at the home. The facility is vendorized by the San Gabriel/Pomona Regional Center.
During the visit today, LPA toured the facility and review the Infection Control Plan. The facility has 4 client bedrooms, 2 bathrooms, living room, dining area, kitchen, and attached garage. The outdoor has a shaded area with table and chairs. Client bedrooms have the required furniture, sufficient lighting, and closet space. There are no items obstructing the walkways. There is no swimming pool on the premises. Facility has operable smoke detectors and a carbon monoxide detector. LPA observed 2 day of perishable and at least a week of non-perishable food items. The hot water temperature is measured within the required range of 105-120 degrees F. Knives and sharps are locked in the kitchen cabinet. Cleaning supplies are stored and locked in the garage.
LPA reviewed the Infection Control Plan. Staff stated they are continuing to follow the guidelines and cleaning everyday.
There are no deficiencies issued today. LPA will return another day to continue the annual inspection. An exit interview was held with A. Fernandez. A copy of this report was provided.
