Facility Evaluation Report

Licensing Program Analyst (LPA) Alberto Lopez conducted an unannounced Case Management visit to follow up on the death of Client #1. LPA met with Guoliang Li, Administrator, and explained the purpose of the visit.

Per information provided by the facility, Client #1 was taken by Staff #1 (S1) to Alhambra Hospital Medical Center to have pre-scheduled surgeries on 06/19/25. Hospital discharged client after the surgeries at 5:00PM against facility wishes to keep C1 overnight. C1 was brought back to facility and expressed pain in her nose and throat and facility staff provided pain medication, changed her bandages and administered nasal spray prescribed by her physician. On the morning of 06/20/2025, (S1) provided C1 with nutritional shake of ensure and apple juice. At around 10:00AM, S1 helped C1 to the bathroom and back to bed. S1 went back to check on C1 shortly after and noticed C1 was in distress and hardly breathing, S1 called 911, C1 was pronounced deceased at 10:30AM. Funeral Home picked up body at 3:00PM. Paramedics did not provide facility with any paperwork. LPA interviewed Administrator and S1
