Facility Evaluation Report

Kristine Panameno, Program Manager called LPA Lopez on February 20th 2024 to report a small fire near the outdoor outlet, there where no injuries or damage according to Program Manager. It has since been repaired. On the day of the incident, Program Manager contacted 911 and was directed to contacted the fire department and was told to evacuate building. Program Manager evacuated the building and sent the participants to the Montebello Shopping center until everything was sorted out by the fire department.Fire department made visit to facility and inspected the location of the incident and cleared the building.

LPA took tour of facility and location of incident and found not risk of safety hazards.
