Complaint Investigation Report
One staff, S#4 was not working during visit and LPA was unable to reached staff via phone but LPA obtained S4 notes from 06/24/2026 and there is no mention of C1 being hit by anyone or having any type of behavior that day.
Several staff stated that C1 has history of making up things. C1 Day Program Annual Behavioral support plan dated 01/06/2026 shows that C1 has history of lying. LPA interviewed three (3) clients and all three (3) where not able to corroborate the allegation. LPA attempted to interview other clients but was not successful due to cognitive issues. LPA observed C1 to be clean and in clean clothes and LPA did not observe any bruises, cuts or injuries on C1 during the visit. There is not enough evidence to substantiate this allegation.
Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
