Complaint Investigation Report

LPA interviewed Licensee and Licensee stated that he thought he was doing the process correctly. Licensee stated that he notified the Department about selling Excel Healthcare Inc but could not provide proof to the Department. One staff member stated staff was aware of the pending sale from another source but was never notified by Licensee. Administrator S2 stated that he was never notified that Excel Healthcare Inc was sold to third party. Review of records from East Los Angeles Regional Center dated July 2, 2025 and State of California Secretary of State form filed on 01/03/2025 show that on 12/31/2025 Excel Healthcare Inc was sold. Excel Healthcare Inc consisted of an Adult Day Care center and Candlelight Home Bartlett (198600875). The department was not notified of the sale of Excel Healthcare Inc on 12/31/2024 and Candlelight Home Bartlett (198600875) was operating without a license from January 1, 2025, until it became licensed under Bartlett Healthcare LLC (198603800) on 04/23/2025. There is sufficient evidence to substantiate this allegation.

Based on LPAs interviews which were conducted and record review, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 6 and Chapter 1 are being cited on the attached LIC 9099D.
