Facility Evaluation Report

Licensing Program Analyst (LPA) Alberto Lopez conducted a subsequent announced Pre-Licensing visit and met with Administrator Gil Calingasen and Doctor Harold Fajardo Licensee for the purpose of conducting a Pre-Licensing Inspection follow up. This is the facility’s first 2nd Pre-Licensing Inspection. LPA discussed the purpose of the visit.

The Pre-Licensing Inspection Tool was used.

LPA inspected the following that required correction.

1) C1 room needs cleaning or painting on one wall – completed

2) C1 door requires locking knob -Completed

3) C2, C3 door needs to close and lock and requires doctor’s orders to have the protective strip on the edge of door - Completed

4) C2, C3 small window needs repair or replacement. - completed

5) Full bathroom vent needs cleaning - completed

6) Full bathroom light fixture requires paint or replacement.- completed

7) Outside needs shade and chairs. - completed

8) Black out curtains in C1 and C2,C3 room, dining room and kitchen. competed

9) C1, C2 needs updated LIC602 - completed
