Facility Evaluation Report

On November 6, 2025, at around 10:30 AM, an office meeting was held to discuss additional civil penalties for a complaint that concluded on October 20, 2022 alleging “Resident developed multiple pressure injuries while in care and Resident sustained a fracture while in care.”. Present at the meeting were Licensing Program Manager (LPM) Ulysses Coronel; Licensing Program Analyst (LPA) Mario Leon; Licensee Leia Joaquin.

During the meeting, the LPM discussed the following:

· Appeal Rights for additional civil penalties

· The department offered the Technical Support Program (TSP)

There have been zero (0) deficiencies cited during today's visit.

An exit interview was conducted and a copy of this report was provided to the Licensee, Leia Joaquin.
