Facility Evaluation Report
Licensing Program Analysts (LPAs) Huma Rahimi and Michael Cava conducted an unannounced Case Management-Incident visit to this facility and met with Day Program Supervisor, James Sauer and explained the reason for the visit.
On 02/06/2025, Community Care Licensing (CCL) received a Report of Suspected Physical Abuse (SOC 341) and Special Incident Report (SIR) from the facility. On 02/05/2025, Client #1 (C1) attempted to grab his/her 1:1 staff #1 (S1) and another staff #2 (S2) reported witnessing S1 slap C1's hand multiple times. S2 immediately reported the incident to one of the supervisors.
The facility immediately removed S1 from the facility after the incident and conducted an internal investigation. Upon completion of the internal investigation, on 02/10/2025, S1 was permanently terminated from their position.
No deficiencies are cited during today's visit.
