At approximately 11:00 a.m. on 10/26/2022, Licensing Program Analyst (LPA) Nicholas Reed conducted an unannounced case management visit. LPA met with staff and disclosed the reason for the visit. LPA toured the facility and did not observe any immediate health or safety concerns.
At 11:13 a.m. LPA interviewed the Administrator.
At approximately 11:30 a.m. LPA interviewed staff and reviewed resident records.
At approximately 11:45 a.m. LPA interviewed Resident #1 (R1).
