Facility Evaluation Report
At 8:30 a.m., the Licensing Program Analyst (LPA) conducted a Case Management visit to confirm the closure of the facility. The LPA met with the Licensee and explained the reason for the visit.
The Licensee initiated the closure due to a change in the facility’s use from a Residential Care Facility for the Elderly (RCFE) to a congregate living health facility. During the visit, the LPA was given a tour of the facility and confirmed that there were no RCFE residents residing in the facility and that operations had ceased. The Department received official written notice of the closure from the Licensee on 01/26/2026; however, the facility requested the closure of the RCFE effective 01/01/2026. The Licensee surrendered the license to the LPA.
The LPA will submit the file for closure upon returning to the Woodland Hills South Regional Office. The LPA will e-mail the closure survey.
An exit interview was conducted, and a copy of this report was signed and delivered.
