Facility Evaluation Report

Licensing Regional Manager (RM) Angela Whittaker, Licensing Program Manager (LPM) Nichelle Gillyard, and Licensing Program Analyst (LPA) Huma Rahimi met with the Administrator/President Vahagn Petrosyan and Nazar Yegeyan, Vice President to conduct an Informal Conference held at the Woodland Hills South Adult and Senior Care Regional Office. The purpose of the informal was to discuss dual licenses, one through Community Care Licensing Division (CCLD), for Residential Care Facility for Elderly (RCFE), effective 06/28/2024, and another license under California Department of Public Health (Congregate Living Health Facility), License # 550009923, effective 05/27/2025.

The facility currently has four (4) RCFE residents and zero (0) Congregate Living residents. The Administrator/President informed RM, LPM, and LPA that he does not intent to retain any Congregate Living residents for at least another eighteen (18) months until the provider number is provided. The Administrator/President also informed that the facility will do a clear transition of the license and let CCLD know a timely manner. The Administrator will eventually surrender CCLD license for RCFE once the transition is complete.
