Facility Evaluation Report

At 2:00 PM , Licensing Program Analyst (LPA) Huma Rahimi conducted an unannounced Case Management – Other visit . LPA met with the staff/Administrator, Florence Perigrino , and explained the reason for the visit.

The purpose of the case management visit was to deliver a copy of the informal letter for a meeting scheduled on January 23, 2026, at 10:00 AM at the Woodland Hills Regional Office , located at 21731 Ventura Blvd, STE 250, Woodland Hills, CA 91364 .

During the visit, LPA was informed that in October 2025 , the facility was sold to Ms. Perigrino . LPA conducted a document review and did not observe any documentation indicating that Community Care Licensing Division (CCLD) or LPA were made aware of this transaction . LPA was informed that a letter of notification will be submitted along with a new application for a license to CCLD .

An exit interview was conducted. A copy of this report was signed and delivered ..
