Facility Evaluation Report
At approximately 10:00 AM, Licensing Program Analyst (LPA) Huma Rahimi conducted an unannounced annual inspection at the facility mentioned above. LPA called the Administrator, Collins Akinsola, to grant access to the facility for the annual inspection. LPA waited outside of the facility. Administrator arrived at 10:20 AM, and LPA was granted access to the facility for the inspection. At 10:25 AM, Physical tour was conducted with the Administrator and LPA observed the following:
Kitchen: At 10:26 AM, LPA observed clean floors, walls, and ceilings in the kitchen and dining area. LPA observed enough supplies of staple non-perishable for minimum 1 week and perishable for 2 days at the facility.
Garage: LPA observed an adequate supply of PPE in the garage along with operable laundry machines. Emergency exits were unlocked and free from obstruction. All knives and cleaning materials were in a locked shed.
Bedrooms: At 10:32 AM, LPA observed total of four (4) bedrooms. Bedroom # one (1) is shared. Bedroom # two (2) and bedroom # three (3) are for private use. Bedroom # 4 is for staff which was at the end of the hall and LPA observed to be clean and locked all the times when not attended. There are no live-in staff in the facility. All bedrooms are properly furnished, clean and have appropriate bedding and linens with furniture in good repair.
Bathrooms: The facility contained 2 bathrooms. One bathroom was inside the shared bedroom. That bathroom contained paper towels, a trash can, and liquid soap under the sink. The other bathroom was near the front entrance and contained paper towels, a trash can, a handwashing instruction sign. LPA observed both bathrooms to be clean and in good repair. At 10:38 AM, hot water temperature measured at 107.4°F.
