Facility Evaluation Report
On 06/23/25 Licensing Program Analyst (LPA) Lorena Casillas arrived at facility to conduct an unannounced one year required annual inspection for this facility. LPA met with Administrator Ugorji Onyike.
LPA arrived at 11:00 am and was greeted and granted access by Administrator. No current covid/flu like symptoms or behaviors. Entrance interview was conducted.
A tour of the physical plant was conducted with the Administrator at 11:15 am. The facility has four (4) bedrooms and three (3) bathrooms currently occupying four (4) clients. One (1) bedroom is used as an office that is locked and inaccessible to clients in care. Currently three (3) of four (4) clients were out in the community and one (1) client was in the living area watching TV.
Infection control: LPA reviewed the facility mitigation plan (approved on 03/09/21) to make sure the licensee was following current infection control recommendations.
Living and dining: At 11:20 am, LPA observed the living room and family room area to be neat and clean along with the dining room. The facility maintains a comfortable temperature at 76°F. The carbon monoxide detector and smoke detectors were tested and observed to be operational.
Kitchen: LPA conducted a tour of the kitchen around 11:30 am and observed a supply of seven-day non-perishable and two-day perishable foods. Frozen foods are properly wrapped and stored. Food storage and preparation areas are clean and inaccessible to pests. Knives are stored in a locked cabinet in the garage. There is one (1) fire extinguisher located in the kitchen. The fire extinguisher was last purchased on 08/15/2025 next to a properly stocked first aid kit.
