Facility Evaluation Report
On 10/11/25, Licensing Program Analyst (LPA) Regina Cloyd conducted an unannounced required – annual inspection and met with the Administrator John Nemeth.
The facility is licensed to serve six (6) residents aged 60 and over, all of which six (6) may be non-ambulatory. It has a hospice waiver approved for six (6). The facility currently has three (3) residents receiving hospice care. The rooms are approved for non-ambulatory/bedridden, and the facility is approved for delayed egress/secured perimeter/secured locked perimeter. The facility is a two-story family home located in a residential neighborhood. The facility consists of six (6) resident bedrooms, five (5) bathrooms, a living room, a dining area, an office/laundry space, and an elevator. The dining room includes a fireplace with a secure metal protective screen cover, ensuring it remains inaccessible to residents. There is a patio/courtyard, shaded areas, an indoor/outdoor activity area on the left side of the house, and a balcony on the second floor. The facility is clean, sanitary, and in good repair.
