Facility Evaluation Report

On June 15, 2026, an office meeting was held to discuss possible additional civil penalties for Complaint 11-AS-20230119162436 regarding the allegation “Resident sustained a fracture while in care”. Attendees included Licensing Program Manager (LPM) Ulysses Coronel, Licensing Program Analyst (LPA) Mario Leon, and Simla Mehta - Licensee/Administrator, Meenesh Mehta - Manager and Jennifer Bobadilla - Assistant Administrator.

There were no deficiencies cited during today’s meeting.

An exit interview was conducted, and a copy of this report was provided to Licensee, Simla Mehta.
