On 10/10/25, Licensing Program Analyst (LPA) Regina Cloyd conducted an unannounced - required one-year inspection visit to the above facility and met with Staff and Administrator Jennifer Bobadilla.
During today's visit, LPA toured the facility and reviewed five resident records.
Due to insufficient time, an annual continuation inspection is required. An exit interview was conducted and a copy of this report was provided to the Administrator Jennifer Bobadilla.
