Facility Evaluation Report
On 11/21/2025, Licensing Program Analyst (LPA) Regina Cloyd conducted an unannounced Required – 1 Year Inspection to the above-named facility and met with Staff and Program Director Shavon Starks. LPA explained the purpose of the visit and were accompanied by Staff inside and outside the facility during this inspection.
The facility is a commercial one-story structure located in a residential neighborhood. It consists of the following: a parking lot (drop off/pick up area), lobby with reception area, main activity room with kitchenette, a shared activity room, two (2) restrooms (men and women), one (1) office, storage closets, and shaded back yard with tables and chairs. The facility is licensed to serve thirty (30) adults of which ten (10) may be non-ambulatory.
Outside grounds were toured and no bodies of water were observed. Walkways around the home were clear of hazards. There are no security bars or weapons on the premises.
