Facility Evaluation Report

On 07/13/2026, Licensing Program Analyst (LPA) Bernadette Allen conducted an unannounced annual inspection. LPA was met by Rhoda Domigo Support Staff who was informed of the purpose of the visit. Christina Atencio Administrator arrived approximately around 10:15AM and she was also informed of the purpose of the visit.

The facility is licensed to serve (6) elderly adults ages 60 and above, of which (1) may be bedridden in room number one (1). Six residents (6) may be non-ambulatory and the facility has an approved hospice waiver for six (6) residents.

The facility is a single-story home, located in a residential neighborhood, which includes five (5) bedrooms, three and half (3.5) bathrooms. There is an office area in kitchen, living room, dining room with mini bar/mini library, small family room. The facility has a shaded front porch, equipped with a ramp. The backyard area is equipment with a shaded porch with a gated pool, and an attached 2 car garage.

At 10:32 AM, LPA Allen and Christina Atencio toured the physical plant, both inside and outside. The pool was inspected, and LPA confirmed that the water area is securely locked and properly gated. There appeared to be sufficient shading and adequate outdoor seating available.

AT 11:00 AM, LPA inspected a total of five (5) bedrooms and 3.5 bathrooms.

The bedrooms had the required furniture which appeared to be in good condition. There was additional linens/ and bedding supplies, there was adequate lighting throughout the facility and storage for the residents’ personal belongings.
