Facility Evaluation Report

At 12:10 PM, Licensing Program Analyst (LPA) Bernadette Allen conducted an unannounced visit to conduct an annual inspection. LPA Allen was greeted by Estela Nicdao Support Staff upon arrival.

LPA introduced herself and explained the purpose of the visit. At approximately 12:45 PM, Teresa Guanlao- Administrator arrived and she was informed of the purpose of the visit.

The facility is licensed to serve (6) elderly adults ages 60 and above, of which (6) can be bedridden. The facility has an approved hospice waiver for (2).

The facility is a single-story structure located in a residential neighborhood and consists of (4) bedrooms, (2-1/2) full bathrooms, shaded back yard, laundry room and a detached garage and a pool in the backyard with required fencing.

At 12:20PM, LPA reviewed three (3) staff files for First Aid/CPR certification, criminal record clearance, training, and health screenings which appeared to be current.

LPA reviewed six (6) residents files for admission agreements, updated physician reports, and needs and services plans which appeared to be current.

The first aid kit and manual were checked and fully stocked the last fire drill was conducted on July 18,2026.

At 1:00PM, LPA observed that the four (4) bedrooms have the required furniture which appeared to be in good repair. The rooms also have adequate lighting, and storage for the residents’ personal belongings
