Facility Evaluation Report

On February 4,2026, Licensing Program Analyst (LPA) Deborah Lee conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Brenda Hernandez/Administrator. LPA explained the purpose of today’s visit. The facility is licensed to serve (6) elderly adults ages 60 and above, of which (5) can be non-ambulatory and (1) bedridden on rooms: 6. The facility has an approved hospice waiver for (6). Currently the facility has (6) residents.

The facility is a single-story structure located in a residential neighborhood. It consists of (5) bedrooms, (3) full bathrooms, shaded back yard, front yard, laundry room and attached 2 car garage.

LPA Lee and the Administrator toured the physical plant. There were no bodies of water or obstructions on the premises. The facility was clean, safe and sanitary.
