The inspector’s account

On 05/12/2026, Licensing Program Analyst (LPA) Jose Anguiano conducted an unannounced annual required visit and met with Administrator Assistant Rose Seludo.

The facility is licensed to serve residents aged 60 and over. The facility is approved by six (6) non-ambulatory residents, of which one (1) may be bedridden. The facility also has a hospice waiver for six (6) residents.

The Department toured the kitchen, dining room, living room, five (5) bedrooms, three (3) bathrooms, garage, backyard, and shaded patio area. Storage areas for kitchen supplies, linens, medications, chemicals, and sharps were observed locked and inaccessible to residents. No bodies of water were observed on the premises. Resident rooms were inspected. Beds and bedding supplies were observed in good condition with adequate lighting and storage space for resident belongings. Bed linens, comforters, and bath towels were sufficiently stocked at the time of visit. Bathrooms were operational. Hot water temperature measured between 107 and 120 degrees Fahrenheit in two (2) bathrooms. The facility was furnished at the time of visit. The kitchen was inspected, and the facility maintained at least two (2) days of perishable food and seven (7) days of non-perishable food supplies. Fire extinguishers were serviced on 03/10/2026. Smoke detectors and carbon monoxide detectors were tested and operational. Medication Administration Records (MAR) were reviewed and observed maintained in order. Infection control practices, visitor/staff/resident screening protocols, and sanitizing stations were observed throughout common areas and restrooms. Required postings, including the activity calendar and menu, were observed posted.

Due to time constraints, an additional visit will be required to complete the annual inspection. An exit interview was conducted and a copy of this report was provided.
