The inspector’s account

On 08/07/25, at 1:00pm, Licensing Program Analyst (LPA) Perry Scott conducted an unannounced annual required inspection visit to Highmore Care Home. LPA met with Josefina Guiao, Administrator, and Bernard Basilan, Co-Administrator, and explained the purpose of today’s visit. The facility is licensed to serve (4) Developmentally Disabled clients ages 18 to 59 (4) ambulatory, of which three (3) may be non-ambulatory in bedrooms #1, 2, and 3. Currently, the home has (4) clients. The clients are Harbor Regional Center clients. None of the clients have Restricted Health Care Conditions and none are utilizing postural supports or protective devices. The facilities annual fees are current.

The facility is a single-story residential home located in a residential neighborhood. The home consists of the following: 4 bedrooms, 2 1/2 bathrooms, kitchen, living room, dining area, laundry area, and a backyard.

LPA conducted a records review of (4) client records, (5) staff records, (3) clients Personal & Incidental Records and reviewed the facility emergency disaster plan. All client and staff records were complete. The facility emergency disaster plan was current and in compliance with Title 22 regulations at the time of visit. LPA reviewed (3) client Medication Administration Records and medication, and did not observe any discrepancies at the time of visit.

At 1:20pm, LPA and staff toured the physical plant. There are no bodies of water or firearm/ammunition on the premises. All client rooms were checked. Beds and bedding were in good condition, adequate lighting provided, adequate storage for client personal belongings was observed. Walls and floors were in good repair. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. Bathrooms were found to be within Title 22 regulations. Toilets and water faucets worked properly. The shower was free of mold/mildew, there is adequate lighting, and sufficient toiletries are accessible to clients. The water temperature measured 118.9F degrees in the kitchen, and 115.6F degrees in the bathroom; a comfortable temperature is maintained in the facility.
