Facility Evaluation Report
On 4/24/25, Licensing Program Analyst (LPA) Mario Leon conducted an unannounced annual required using the CARE Inspection Tool. LPA met with staff one Ceceni Rose Pare, House Manager (S1). LPA explained the purpose of today’s visit. The facility is licensed to serve (6) elderly adults ages 60 and above. Facility has an approved hospice waiver for (6).
The facility is a single-story structure located in a residential neighborhood. It consists of six (6) bedrooms, two (2) full and one (1) half-bath (1/2 bath), shaded back yard, front yard, laundry room in the attached 2 car garage.
LPA toured the physical plant with S1. There were no bodies of water or obstructions on the premises. A total of (8) rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, and ample storage for the residents’ personal belongings was observed. The water temperature ranged from 115-120°F. Bathrooms were found to be within Title 22 regulations and were fully operational. The facilities' indoor temperature was at a comfortable temperature, from 76F° – 78F°. LPA inspected (4) rooms and (3) bathrooms. Smoke and carbon monoxide are all operable conditions.
LPA observed the facility to be sanitary and appropriately furnished at the time of the visit. Storage areas for personal hygiene were observed. Cleaning supplies, toxins, and sharps objects were stored and not accessible to residents in care. The kitchen was inspected and there is sufficient perishable and non-perishable food and all foods were maintained properly. All fire extinguishers were charged and were operable. The last Fire/Disaster Drills were conducted on 3/10/25. LPA observed "1-844-LET-US-NO" (PUB475) was posted as required, yet was observed in the 8.5 inches x 11 inches format.
