Facility Evaluation Report
On 7/8/2026, Licensing Program Analyst (LPA) Bernadette Allen arrived at facility to conduct an unannounced annual inspection. Upon arrival LPA met with Administrator Oscar Lechuga and he explained the purpose of the visit
The facility is licensed to serve six (6) residents ages 60 and over. The facility has been approved for six (6) non-ambulatory residents. Approved to retain one (1) Hospice Resident. Facility has an approved hospice waiver for (6). Fees are not current and will be paid by July 31,2026.
The facility is a one-story home located in a residential area The house consists of 3 bedrooms, 1 staff bedroom, 2 bathrooms, 1 dining/living room, kitchen,attached garage, front yard and backyard patio area with shaded seating.
At 2:55 PM, LPA toured inside and outside grounds and no bodies of water were observed. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident. Resident bathrooms were observed to have working toilets and water faucets, and grab bars were secure.
At 3:15 PM, LPA reviewed two (2) staff files for First Aid/CPR certification, criminal record clearance, training, and health screenings which all appeared to be current.
LPA reviewed one (1) residents files for admission agreements, updated physician reports, and needs and services plans which all appeared to be current.
